How your health information is used and protected

This notice describes how medical information about you may be used and disclosed and how you can get access to this information.

Grand Junction Dermatology is required by law to maintain the privacy of your protected health information (“PHI”), to provide you with this notice describing our legal duties and privacy practices, and to follow the terms currently in effect.
How we use your information
- For treatment — We share information among our providers and staff, and with other physicians, labs, or pharmacies involved in your care.
- For payment — We use and share your information to bill and collect payment from you, your insurer, or a third party.
- For health care operations — We use information internally for quality assessment, staff training, licensing, and normal business operations.
Other permitted uses and disclosures
- As required by law, or to avert a serious threat to health or safety.
- For public health activities and health oversight audits.
- To report suspected abuse, neglect, or domestic violence.
- In response to court orders, subpoenas, or law enforcement requests.
- For organ and tissue donation, research, and workers’ compensation.
- To coroners, medical examiners, and funeral directors as needed.
- With family members or others involved in your care, unless you object.
Uses requiring your written authorization
Uses not described above, including most marketing communications, the sale of your information, and psychotherapy notes, require your written authorization. You may revoke that authorization in writing at any time.
Your rights
- Get a copy of your paper or electronic medical record.
- Ask us to correct your medical record.
- Request confidential communications.
- Ask us to limit what we use or share.
- Get a list of those with whom we’ve shared your information.
- Get a copy of this notice, even if you agreed to receive it electronically.
- Choose someone to act on your behalf.
- File a complaint if you feel your privacy rights have been violated.
Our responsibilities
We are required by law to maintain the privacy and security of your protected health information, notify you promptly if a breach occurs, follow the duties described in this notice, and not use or share your information other than as described here unless you tell us otherwise in writing.
We reserve the right to change the terms of this notice, and any revised notice will apply to information we already have as well as information we receive in the future.
A copy of the current notice will always be posted at the front desk.
